Breast augmentation is a popular procedure in our practice for women who want larger breasts with better shape. Women with a moderate degree of breast sagging and smaller breasts can frequently benefit by augmentation as can those who have asymmetry of breast size and shape. An attractive result from this procedure provides a more positive self image and confidence to many patients.

Breast Augmentation: The Procedure

The procedure is done on an outpatient basis under general anesthesia. A small incision is made under the breast, around the nipple, or under the arm and an implant is inserted under the breast tissue and the underlying pectorals major muscle. Sometimes the implant is placed on top of the muscle, but most patients in our practice have augmentation behind the muscle (subpectoral position). By going behind the muscle, we reduce the risk of capsular contracture, (a tightening of the soft tissue capsule around the implant which can cause it to feel hard or look distorted) and can avoid visibility of the implant shell in the cleavage area. We also reduce interference with mammograms and the risk of surface rippling.
Skin incisions are closed with absorbable sutures under the surface that do not have to be removed. Saline implants are used most commonly. Silicone gel implants may be used in appropriate patients who meet criteria for inclusion in ongoing clinical trials. Gel implants are likely to become widely available to all patients who desire them in the near future pending FDA final approval. The date for this change is not known at this time (September, 2008).

Breast Augmentation: After Surgery

After surgery and the recovery room, you are discharged home. There is a light bandage in place and a soft bra. Discomfort is controlled with oral medication including a muscle relaxant and a pain medication. Antibiotics are prescribed for one week. Patients are seen one day after surgery to insure they are doing well. They may shower on the second day.  Massage and exercises to avoid capsular contracture are recommended at appropriate intervals.  An elastic bandeau (strap) may be used for several weeks to help the implants settle appropriately in the created pocket. Light activities can be resumed as tolerated.

Most patients return to work or school in 4-5 days. Exercise is resumed on a gradual basis, avoiding heavy lifting or workouts for the first 3-4 weeks. Avoiding use of underwire bras for 6 weeks and not shaving underarm incisions for three weeks are recommended. The breasts will feel firm right after surgery, and will soften and appear more natural as healing takes place over the first year.

Breast Augmentation: Major Complications are Rare

All breast augmentation procedures have some degree of risk. While most patients do not have complications, minor complications that do not affect the outcome of a treatment or surgery may occur after some procedures. Major complications are rare. The specific risks and the suitability of a particular procedure are carefully reviewed at your consultation (and preoperative visits if you are having surgery).  Your safety and comfort are our foremost concerns.
