Thank you for your interest in our practice. We request that you provide your mailing address so that we may send out a practice information packet directed at your interests. We request your telephone number so that we may best assist you in scheduling your consultation.

Consultations are charged $50.00 at the time of booking by credit card. This is applicable to your surgery fees. If you need to cancel for any reason, we ask that you contact us at least 48 hours in advance, and we will promptly refund your consultation fee. This allows us to use that time slot for another patient. If you just don't show up or do not cancel with 2 days notice, the $50.00 fee is forfeited. If you need to reschedule, you will not be charged an additional fee on a one time basis. If you then cancel a second time, you will forfeit your consultation fee and a second consultation fee would apply if you wish to reschedule at that point.
